If you have dental insurance, please bring your insurance card or provider details so we can assist you with claims and coverage information.

3. Medical History

Please provide any relevant medical records or a list of health conditions. This includes conditions like diabetes, high blood pressure, or any special health needs, as this information helps us personalize your care.

4. List of Medications

Bring a list of any medications you're currently taking, including over-the-counter drugs and supplements. This helps us manage your treatment and avoid any complications with medication interactions.

5. Completed New Patient Form

If this is your first visit, please complete our new patient form in advance. You can download it from our website or fill it out when you arrive. This form helps us gather necessary information about your medical and dental history.

6. Payment Information

Please bring any payment methods (credit card, cash, or check) for co-pays, deductibles, or treatments not covered by insurance.

7. Comfort Items (Optional)

If you have any items that help you feel more at ease, such as a favorite blanket or headphones, feel free to bring them along. We’re here to make you feel as comfortable as possible during your visit.
